Special Instructions

New clients are asked to arrive 10 to 15 minutes before your appointment to complete any required paperwork if you haven’t already done so online. Please attend your appointment alone, as only the Esthetician and client are permitted in the treatment room for insurance reasons. This policy also includes not bringing children. To maximize your treatment, we recommend coming without makeup as this takes away from your full treatment. Wear comfortable clothing and bring some cozy socks, as you’ll need to remove your shoes. You’re also welcome to bring a personal blanket if you like since our room can be a bit chilly despite the blanket we provide.


Cancellation policy

In order to provide high-quality care within a reasonable timeframe, we have implemented a cancellation policy.

Appointments are in high demand, and cancelling in advance allows us to offer the time slot to another client in need of timely care. This policy helps us keep our schedule running smoothly and ensures availability for everyone.

We understand that unexpected situations can happen. If you need to cancel or reschedule your appointment, please notify us at least 24 hours before your appointment time.
For example: If your appointment is on Wednesday at 3:00 PM, you must cancel by Tuesday at 3:00 PM to avoid any fees.

If less than 24 hours’ notice is given, a 50% cancellation fee of the scheduled service will be charged.
If you fail to arrive at your scheduled appointment without notifying us, a 60% fee of the scheduled service will be charged.

Please note: Arriving more than 15 minutes late will be considered a no-show, your appointment will be cancelled, and the same fee will apply.

Client Consent form

I hereby consent to and authorize Morgan Zion Dutro to perform the following procedure.

I have voluntarily chosen to undergo this treatment/ procedure after the nature and purpose of this treatment has been explained to me, along with the risk and hazards involved.

While it is not feasible to enumerate every potential risk and complication, I have received information regarding potential advantages, risks, and complications. I also acknowledge that there are no guaranteed outcomes, and individual results depend on factors such as age, skin condition, and lifestyle. It is possible that further treatments may be necessary for the desired results, incurring additional costs.

I have thoroughly read and comprehended the instructions for post-treatment home care. I understand the significance of adhering to all given instructions for post-treatment care. If I have any further inquiries or concerns regarding my treatment or the recommended home products/post-treatment care, I will promptly consult the esthetician.

Photo & Video release form

I hereby grant and authorize Morgan Zion Dutro the right to capture, modify, edit, reproduce, exhibit, publish, distribute, and utilize any photographs, videos, and/or audio recordings taken of me for lawful promotional purposes. These materials may include, but are not limited to, newspapers, flyers, posters, brochures, advertisements, press kits, websites, social media platforms, and other forms of print and digital communication. I provide this authorization without expecting any payment or other forms of consideration.

This authorization remains in effect indefinitely and applies to all languages, media, formats, and markets, whether currently known or discovered in the future.

I willingly waive any rights to royalties or other compensation arising from or related to the use of these photographs or recordings.

I acknowledge and accept that the materials created through this agreement will be the property of Zion Radiance Esthetics and will not be returned to me.

I hereby release and discharge Zion Radiance Esthetics from any liability, claims, or legal actions that may arise, including those made by myself, my heirs, representatives, executors, administrators, or any other individuals acting on my behalf or on behalf of my estate.

By checking the agreement box and signing below, I confirm that I have read and fully understand all Terms & Conditions listed above. I agree to the Special Instructions, Cancellation Policy, Client Consent Form, and Photo & Video Release guidelines, and acknowledge that my signature serves as acceptance of all included policies.
